The following items are required to complete an application for admission:

  $100 Application fee for Academic Year
  $150 for Summer Camp
  $250 for Academic Year and Summer Camp

• Report of Medical History (word file) or (PDF file) or (Spanish PDF) (with physician's signature)
• Consent & Insurance Form (word file) or (PDF file) or (Spanish PDF)
• Complete Immunization History (required by state law)
• Medical Addendum Form (word file) or (PDF file) or (Spanish PDF)
• Copy of Birth Certificate
• Transcript Request Form (word file) or (PDF file) or (Spanish PDF)with Comprehensive and Current Official School Transcripts and most recent report card showing semester work in progress
• Principal/Counselor Evaluation Form (word file) or (PDF file) or (Spanish PDF)
• English Teacher's Evaluation Form (word file) or (PDF file) or (Spanish PDF)
• Math Teacher's Evaluation Form (word file) or (PDF file) or (Spanish PDF)

Upon receipt of these forms, the Admissions Committee will review the file. The Committee's decision is based on the contents of the file alone unless the applicant has toured the Academy and met with key academic, military and administrative personnel. Enrollment at the Academy is limited. Should we receive applications with similar or equal credentials, slots are awarded on a first-come, first-served basis.
