Application For Admission (word file) or (PDF file) (complete front & back)
$100 Application fee for Academic Year
$150 for Summer Camp
$250 for Academic Year and Summer Camp
Report of Medical History (word file) or (PDF file) (with physician's signature)
Consent & Insurance Form (word file) or (PDF file)
Complete Immunization History (TX state law requires complete history)
Medical Addendum Form (word file) or (PDF file)
Copy of Birth Certificate
