  Application Fees:
  $100 Application fee for Academic Year
  $150 for Summer Camp
  $250 for Academic Year and Summer Camp

• Report of Medical History (word file) or (PDF file) or (Spanish PDF) (with physician's signature)
• Consent & Insurance Form (word file) or (PDF file) or (Spanish PDF)
• Complete Immunization History (required by state law)
• Medical Addendum Form (word file) or (PDF file) or (Spanish PDF)

Additional forms to complete for Academic Year Applications:

Academic Year Applications: The Admissions Committee will review an applicates file upon receipt of the above forms. The Committee's decision is based on the contents of the file alone unless the applicant has toured the Academy and met with key academic, military and administrative personnel. Enrollment at the Academy is limited. Should we receive applications with similar or equal credentials, slots are awarded on a first-come, first-served basis.
